Subcision vs TCA CROSS for Acne Scars: Match the Method to the Scar

They treat different scar shapes, so the real question is which scars you have. Many people have more than one type.

    Quick verdict

Subcision is for rolling scars that are pulled down by bands under the skin: a needle is passed under the scar to release them. TCA CROSS is for narrow, deep ice-pick pits: a doctor places a strong acid into each pit to prompt it to fill in from the bottom. Neither is the universal winner. The American Academy of Dermatology notes that ice-pick and rolling scars need different treatment and that dermatologists often use more than one method, so if you have both scar types, a combined or staged plan is common. Choose a doctor who can map your scars and explain why each method is used where.

    Why they are often combined

Many people have a mix of scar shapes. Subcision lifts a tethered rolling scar but does not close a narrow deep pit; TCA CROSS can help a pit fill in but does not release bands under a rolling scar. That is why doctors may stage them, or add other treatments such as microneedling or fillers. The American Academy of Dermatology gives the example of lifting sunken scars surgically and then adding a filler for volume.

    What the evidence says

A small 2011 split-face study in 20 people with rolling scars and skin types III–IV found more improvement on the subcision side than with 100% TCA CROSS, and more pigment change on the TCA side. That was for rolling scars only, which TCA CROSS is not mainly aimed at, so it does not show which is better for ice-pick scars or for everyone.

    Cost

Published examples, checked 27 Sep 2026: Hebe Clinic lists TCA CROSS from around RM500 per session, depending on the number and severity of scars and the area. Nexus Clinic KL lists subcision at RM500–RM900 per session for 2026, with the final price set after consultation. These are single-clinic prices, not a Malaysian average. Both methods are often repeated or combined with other treatments, so ask for the planned number of sessions and the total before you start.

    Which should you choose

Ask the doctor to show you which of your scars are rolling, ice-pick or boxcar, and which method they plan for each. Subcision makes sense for rolling scars that look pulled down from underneath. TCA CROSS makes sense for narrow, deep pits. If you tan easily or have had dark marks after spots or procedures, ask how pigment risk will be managed, especially for TCA CROSS. A good plan names the scars, the methods, the number of sessions and what you will pay in total.

    What to verify

Ask who performs each procedure and check the doctor through the official Malaysian registers. Never buy TCA or attempt subcision yourself. BookAClinic has not verified the clinics mentioned here.

    Frequently asked questions

Subcision is the method aimed at tethered rolling scars. A small 2011 study in rolling scars found more improvement with subcision than with TCA CROSS, and more pigment change with TCA CROSS.
