Microneedling vs RF Microneedling for Acne Scars: What Changes?
RF microneedling adds heat to the needles. That can help some scars, but it also adds device and settings risk. It is not automatically the better choice.
Quick verdict
Both create tiny controlled injuries to prompt new collagen; RF microneedling also heats the skin through the needles. The American Academy of Dermatology says combining RF with microneedling can make scars less noticeable than either alone. It describes both as options for all skin tones, which is a general statement, not a guarantee for you. In practice, which one suits you depends on your scar types, your skin, the exact device and the person operating it. RF sessions usually cost more, and the US FDA reported serious complications with certain RF microneedling uses in 2025. Choose the plan your doctor can explain for your scars, not the one with more technology.
How the two compare
| Microneedling | RF microneedling | |
|---|---|---|
| How it works | Fine needles make tiny punctures that prompt the skin to make new collagen and elastin. | The same punctures, plus radiofrequency heat delivered through the needles into deeper skin. |
| Scars it is often used for | Overall texture and shallow, spread-out scarring; often combined with other treatments. | Often offered for rolling and boxcar scars and texture. Narrow, deep ice-pick scars usually need a different treatment such as TCA CROSS. |
| Skin tone and pigment | AAD describes it as an option for all skin tones, but darker marks after treatment are still possible; ask how they are prevented for your skin. | AAD describes it as an option for all skin tones, but heat and settings still carry risk, and the FDA has reported serious complications with some RF uses; ask how they are chosen for your skin. |
| Discomfort and downtime | Usually numbing cream; expect redness afterwards. Ask how long for your plan. | Usually numbing; redness and swelling afterwards. Ask how long for your plan. |
| Device and operator risk | Sterile technique, needle depth and skin preparation matter. Do not use penetrating needles at home. | In 2025 the FDA reported burns, scarring, fat loss, disfigurement and nerve damage with certain RF microneedling uses. Device choice, settings and training matter. |
| Published price examples | AOKLINIK, Penang: medical microneedling from RM1,250 per session. | AOKLINIK, Penang: from RM1,899 per session. Nexus Clinic, KL (2026): RM1,000–RM1,800 per session. |
What the evidence says
A 2025 systematic review of fractional RF microneedling used alone for acne scars included 16 studies with 481 patients. It concluded RF microneedling is likely effective as a single treatment, but said more randomised trials are needed to establish treatment settings. It did not show that RF microneedling beats plain microneedling for everyone.
The FDA's October 2025 safety communication describes reported serious complications with certain RF microneedling uses. It does not give a rate, so it cannot tell you how often this happens; it is a reason to ask about the device, settings and operator.
Cost
Published examples, checked 27 Sep 2026: AOKLINIK Penang lists medical microneedling from RM1,250 and RF microneedling from RM1,899 per session (page last reviewed 26 Aug 2026). Nexus Clinic KL lists RF microneedling at RM1,000–RM1,800 per session for 2026, with the final price set after consultation. Neither clearly names the device or area for each price, so they are not a Malaysian average or like-for-like. Budget for a course, not a session: AOKLINIK says a mixed scar-treatment course of four to eight sessions at its clinic commonly runs RM3,000–RM9,000; that is not a price for RF sessions alone. Ask for a written plan with the number of sessions, device, area and total.
Which should you choose
Ask the doctor to tell you which scar types you have (rolling, boxcar, ice-pick or mixed) and why the proposed treatment suits them. Microneedling can make sense for general texture or if you want a lower cost per session. RF microneedling can make sense for rolling or boxcar scars when the doctor can name the device, explain the settings for your skin and describe what happens if you get a burn or darker marks. Ask what the full course costs, and whether a combined plan would suit your scars better.
What to verify
Ask for the exact device brand and model and check it with Malaysia's Medical Device Authority; US FDA information is not Malaysian registration. Ask who will perform the treatment and how complications are handled. BookAClinic has not verified the clinics or devices mentioned here.
