How many sessions of under-eye filler for dark circles?

A straight answer, grounded in ASPS guidance and peer-reviewed reviews: most patients need 1 to 2 sessions of hyaluronic acid tear trough filler, spaced 2 to 4 weeks apart. But filler only works for one of the three causes of dark circles. Find out which one you have first.

    The Protocol

What a typical tear trough filler plan looks like

Aesthetic doctors in Malaysia follow the same conservative staged protocol used in ASPS and peer-reviewed guidance. Under-fill, review, top up if needed.

    Session 1

    Assessment and initial correction

    Volume

    0.5 – 1 ml total

    When

    Day 0

Full periorbital assessment (tear trough depth, pigment, vasculature, skin quality). Conservative injection of 0.5 to 1 ml HA filler, both sides. Ice, light activity for 24 hours.

    Review

    2 to 4 week review

    Volume

    No product

    When

    Week 2 – 4

Swelling has fully settled and the true result is visible. Doctor decides whether a second session is needed for residual hollowing, asymmetry, or deeper tear troughs.

    Session 2 (if needed)

    Top-up and refine

    Volume

    0.25 – 0.5 ml

    When

    Week 4 – 6

Roughly 40 to 60 percent of patients need a second session, typically 0.25 to 0.5 ml, to complete correction. Placement is refined based on how session 1 settled.

    Maintenance

    Annual top-up

    Volume

    0.5 ml / year

    When

    12 – 18 months

Results last 12 to 18 months. Most patients maintain with a single 0.5 ml top-up per year rather than restarting from scratch.

This is a general protocol drawn from published ASPS and Aesthetic Surgery Journal literature. Your exact plan must be built by an LCP-certified doctor after a physical assessment.

    Diagnosis First

Which cause of dark circles do you have?

Filler works beautifully for volume loss and does nothing for pigment or vascular causes. Diagnose first, treat second.

    Regulation

Tear trough is a high-risk area, verify your doctor

Under the MOH Aesthetic Medical Practice Guidelines, every doctor injecting tear trough filler must hold specific credentials. These are all publicly checkable.

    MMC Annual Practising Certificate

The Malaysian Medical Council issues an annual practising certificate to every registered medical practitioner. Without it, they legally cannot practise medicine in Malaysia.

    MOH LCP for tear trough

The Letter of Credentialing and Privileging specifies which aesthetic procedures the doctor is authorised to perform. Ask specifically that dermal filler in the periorbital area is on their LCP.

    NPRA-registered filler

The exact HA product used must be NPRA-registered. Ask to see the box and batch number. Common tear trough options: Restylane Eyelight, Teosyal Redensity II, Belotero Balance.

BookAClinic runs Malaysia's only free public lookup that combines the MOH National LCP Registry and MMC records in a single search. Check your doctor by name, LCP number, or MMC number, before you book.

    Before You Book

Red flags to walk away from

Tear trough is one of the highest-risk injection areas on the face. If a clinic does any of the following, do not book.

Cannot show its MOH facility registration certificate on request.

Refuses to name the exact HA brand and batch number being injected.

Advertises tear trough filler under RM 1,200 total, real product cannot be legitimately sold at that price.

Wants to inject 2 ml or more in a single session, greatly increases risk of puffiness and Tyndall effect.

The injector is a beautician, therapist or 'skin specialist' without LCP for periorbital filler.

Offers to inject you immediately without a proper diagnosis of whether your dark circles are actually volume-related.

No plan for hyaluronidase (reversal enzyme) or emergency vascular occlusion protocol on-site.

    FAQ

    Tear trough filler, answered

For most patients, tear trough filler is a staged treatment across 1 to 3 sessions, spaced 2 to 4 weeks apart, with a touch-up review at 4 to 6 weeks. Roughly 40 to 60 percent of patients achieve their goal in a single session using 0.5 to 1 ml of hyaluronic acid total (both sides). Patients with deeper tear troughs, more pronounced volume loss or hollowing typically need 2 sessions. A third session is uncommon and usually reserved for revision or asymmetry correction. After the initial protocol, most patients maintain results with a single top-up every 12 to 18 months.
